Use this guide to add and configure questionnaires within a Client Portal request in Mercury Nexus. Questionnaires allow you to collect structured responses from clients, including needs analysis and compliance-related information.
You must configure questionnaires while the request is in Draft status.
Access the Questionnaires module
Open the relevant Opportunity.
Select Client Portal from the side navigation.
Double-click the request to open it.
Select Request Builder from the left-hand panel.
Locate Questionnaires.
Enable the module using the toggle on the right.
Add a questionnaire
Click + Add Questionnaire.
Select the required questionnaire from the list.
Confirm your selection.
The questionnaire will appear within the module.
Understand shared and individual modules
Each questionnaire may contain:
Shared modules – completed once per request
Individual modules – completed separately by each applicant
Shared modules are enabled using a toggle.
Individual modules are assigned using checkboxes beside each applicant’s name.
This allows you to control:
Which sections apply to the whole application
Which sections must be completed by each applicant
Enable or disable questionnaire sections
Within the questionnaire:
Toggle Enable module on for shared sections you require.
Tick the checkbox beside each applicant for individual sections.
Only enabled sections will appear in the Client Portal.
Disable sections that are not relevant to reduce client workload.
Remove a questionnaire
While the request is in Draft:
Locate the questionnaire card.
Select the delete icon.
Confirm removal.
The questionnaire will no longer form part of the request.
Understand how clients complete questionnaires
When the request is sent:
Each applicant receives their own dedicated portal link.
They see only the sections assigned to them.
Shared sections appear once per request.
Individual sections appear per applicant.
Completion contributes to the overall request progress percentage.
What you cannot change after sending
Once the request moves out of Draft status:
Questionnaires cannot be added
Questionnaires cannot be removed
Sections cannot be enabled or disabled
Applicant assignments cannot be changed
If changes are required:
Close the request, or
